Please follow the below instructions as Philadelphia City Hall Department of Records is operating by appointment only still as of June 8, 2021 for recording notary bonds. City Hall Department of Records will reopen fully and not require an appointment starting July 6, 2021.
- Print the executed Surety Bond and Power of Attorney received from our office.
- Attach together with a staple.
- Sign as the principal and have your signature witnessed to the left. Blue ink preferred.
- Schedule an appointment with City Hall by emailing email@example.com
Appointment hours: Monday through Friday, 8 a.m. to 2 p.m.
"In-person services at the Department of Records are currently by appointment only. Email firstname.lastname@example.org to schedule."
